How do I prepare for the evaluation? For parents:
Please bring any school or medical records that you may have as well as a list of prescription medications your child is current taking.
Children and adolescents who are not experiencing normal development or who have a medical, genetic, neurological condition that may be interfering with their development
If your child is on medication, please adhere to his or her regular schedule for the day of the evaluation.
If you will be seeking reimbursement from your insurance company, please be sure to contact the insurance prior to your first appointment so that any required forms may be completed.
For children and teen-agers:
Be sure to get a good night sleep.Keep your normal morning routine and be sure to eat breakfast or lunch.
Make sure to wear your contacts and/or bring your glasses with you.
If you use hearing aids, make sure to wear them too.
If there is something else that you use to hear and/or see at school, be sure to bring those with you too.
